Employee Giving Form
As an Arroyo Grande Community Hospital employee you demonstrate generosity and kindness every day. By participating in the Employee Giving Campaign you can give together to support all the ways we are making our community healthier. Through your generosity you can care for your colleagues and join them in building a healthier community for each other.
Employees can use the below form to sign up for payroll deduction gifts (one-time or recurring) or Paid Time Off (PTO) hours. Please note that if you are donating PTO hours, you must have a minimum of 80 hours in your PTO bank.
